It was probably done in her late teens/early 20s. I think it's great that she obviously doesn't do Botox and prefers her face to have movement! And this photo you posted from her appearance at the Stella McCartney show looks indeed very strange and different - look at other recent pics, she doesn't look like that on any other photos! She just got pregnant at that time, maybe she just used a different kind of make-up.. This photo is strange indeed, and looks completely different to all of the others.
