I'm pretty sure they don't put you involuntarily into a psych ward unless you're considered a harm to your self or others. I wonder if she "pulled a Britney". As for a drug problem, meth/coke/etc. can make you look exhaused (bec of no sleep obviously) but it's awfully hard to gain weight on them. See Lohan for reference...
